New Report Supports Medical Home Model
The Patient-Centered Primary Care Collaborative has issued a landmark report, documenting that patient centered medical home models can improve care and save money.
Outcomes of Implementing Patient Centered Medical Home Interventions: A Review of the Evidence from Prospective Evaluation Studies in the United States was released, December 13, in Washington, DC at a briefing co-sponsored by The Commonwealth Fund and the Alliance for Health Reform.
The brief outlines clear findings that the medical home model results in improved quality of care and patient experiences, and that improvements in preventive, coordinated care yield reduced cost from hospital and emergency department utilization.
