Ohio Seeks Grant to Expand Medicaid PCMH Models
The State of Ohio has applied for a federal State Innovation Model (SIM) grant to improve the overall health system performance by engaging public and private payers in statewide implementation of patient-centered medical homes (PCMH) and episode based payment models.
The grant application proposes to: (1) expand the capacity and availability of qualified medical homes to most Ohioans across Medicaid/CHIP, Medicare, and commercially insured patients within a 3-5 year timeframe; and (2) define and administer episode based payments for acute medical events in the same period.
If approved, Ohio would conduct a nine-month project, beginning this year, to engage public and private sector stakeholder by formalizing a comprehensive plan, finalizing design of the PCMH and episode based payment models; and complete SIM planning at an estimated cost of $4.l million from the state and private stakeholders and $3 million from the Center for Medicare & Medicaid Innovation.
