New Report Supports Medical Home Model

The Patient-Centered Primary Care Collaborative has issued a landmark report, documenting that patient centered medical home models can improve care and save money.

Outcomes of Implementing Patient Centered Medical Home Interventions: A Review of the Evidence from Prospective Evaluation Studies in the United States was released, December 13, in Washington, DC at a briefing co-sponsored by The Commonwealth Fund and the Alliance for Health Reform.
